Back Pain Q & A
What are the causes of back pain?
Back pain can arise for numerous reasons, from the effects of aging and repetitive strain to acute trauma. Some likely causes of back pain include:
- Muscle strains
- Ligament damage
- Herniated discs
- Spinal stenosis
- Degenerative disc disease
- Scoliosis
- Facet joint arthritis
- Vertebral compression fractures
- Spondylolisthesis
- Lumbar radiculopathy (sciatica)
Back injuries tend to occur when you overexert yourself, either by doing too much or pushing yourself beyond your fitness levels. Employing incorrect techniques when lifting, twisting awkwardly, and using poor posture can all trigger back pain.
How is back pain treated?
Most cases of back pain are treatable using conservative methods, typically a combination of:
- Activity modification
- Physical therapy
- Medication
- Heat and ice
- Massage therapy
- Stretching and strengthening exercises
- Regenerative medicine
Long periods of bed rest aren't advisable for most causes of back pain, as you're likely to become less mobile without moderate exercise.
Dr. Attias aims to find a combination of therapies that work for you while avoiding as far as possible the use of powerful opioid prescription pain medications.
What treatments are there for persistent back pain?
If your back pain doesn't respond to the more conservative treatment options, Dr. Attias offers advanced therapies, including:
Spinal injections
Epidural and facet joint injections typically contain a local anesthetic and a steroid (cortisone), a potent anti-inflammatory drug. The anesthetic offers a short period of complete or almost complete pain relief, and the steroid reduces inflammation and brings longer-term pain reduction.
As well as cortisone injections, Dr. Attias uses facet and medial branch blocks and sympathetic blocks, which contain a local anesthetic. These injections can be useful in diagnosing the cause of back pain as well as treating it.
Radiofrequency neurotomy
Radiofrequency neurotomy or ablation involves a needle in your spine that delivers radiofrequency energy to the nerves causing your back pain. The heat kills the pain nerves and reduces your discomfort, although the nerves eventually grow back.
Implantable devices
There are two main types of implantable devices. A spinal cord stimulator uses electrical impulses to disrupt the pain sensations your spinal nerves send to your brain. An intrathecal pump delivers pain medication directly to the source of your back pain.
Minimally invasive spinal procedures
Dr. Attias can perform several procedures for back pain, including minimally invasive lumbar decompression (MILD), sacroiliac joint fusion, and the fitting of Vertiflex™ interspinous spacers for spinal stenosis.
